The Stroke Coordinator RN will:
• Contribute to the data management required for the Primary Stroke Center.
• Collect and record data on quality indicators for all stroke patients.
• Assist Primary Stroke Center with data management and education.
• Participate in the Joint Commission and State stroke center certification efforts through data collection, education & submission of forms & other pertinent information.
• Coordinate the multidisciplinary stroke steering committee.
• Collaborate with outreach to the community and Primary Stroke Centers.
• Participate in community and educational outreach activities (including EMS).
• Work with comprehensive stroke centers, to ensure smooth transfer of patients if needed.
• Participate in patient care activities on a stroke designated unit(s):
• Act as an expert resource clinical practitioner and role model in caring for and assisting with planning, implementing and evaluating care of stroke patients.
• Complete daily rounds on inpatient service to ensure continuity of care and compliance with Get With The Guidelines/Joint Commission measures.
• Participate with Stroke Team to facilitate treatment of patients with thrombolytic drugs.
• Participate in patient and family education activities on a stroke designated Unit(s):
• Provide verbal and written instructions to stroke patients and their families as needed.
• Develop educational programs for patients and families as needed.
• Reinforce need for lifestyle modification to patients and families after discharge from hospital and maintains a continuum of care as needed.
• Participate in continuing education programs for nursing and other clinical specialties:
• Identify continuing education needs of all clinical staff, coordinates programs and activities to meet these needs.
• Participate in ongoing training sessions, national and local activities and meetings.
• Share and apply current research and literature in specialty area in direct care, educational, consultant and resource roles.
• Utilize research and educational affiliations to support and advance professional practice.
Requirements:
• Licensed as a Registered Nurse in the State of New Jersey
• BSN preferred or must be obtained within three (3) years of hire date
• Three (3) to five (5) years acute care experience preferred
Required Certifications:
• BLS certification required, as approved by the American Heart Association
• ACLS certification required or obtained during clinical orientation
FT Days
We offer competitive base rates that are determined by many factors, including job-related work experience, internal equity, and industry-specific market data. In addition to base salary, some positions may be eligible for clinical certification pay and shift differentials.
The salary range listed for exempt positions reflects full-time compensation and will be prorated based on employment status.
