Care Coordination

The Social Worker MSW will:
Provides clinical social work services and discharge planning evaluation to medical specialty areas, as appropriate to the ages of the patients and to the developmental life cycle stage. Provides psychosocial support, counseling, assessment and interventions to promote emotional wellness and improved coping. Assists patients and families with adjustment to the social/emotional effects of illness. For identified patients, provides discharge planning evaluation, development and implementation of the discharge plan in conjunction with the patient, family, guardian, physician and other interdisciplinary staff. Demonstrates skills in specific areas with respect to both clinical social work and care coordination/discharge planning functions as applicable including behavioral health, maternal child health, adult medical surgical, geriatrics, care transitions and ambulatory care.

Requirements:
• MSW from accredited Graduate School of Social Work. NJ Social Work License: LCSW or LSW.
• NPI and Medical Non Billing Provider Number are required; the application should be submitted to the State within the first 30 days of employment.
• Previous hospital experience preferred. Experience in medical social work/perinatal pediatrics/behavioral health/case management preferred.
• Position includes some weekend and holiday coverage.
